Hi Peter, this is precisely why I believe that fatal / all accidents should be publicized and analyzed.
Yes they are unpleasant to deal with, but at least if we learn everything we can from the circumstances that lead to the accident, people can identify the risks that they are dealing with and prevent a repeat.
This at least slightly mitigates the original loss of life and ensures it wasn't just a complete waste!
Greem's comment regarding the involvement of the legal people is sadly only too accurate, there needs to be some sort of no fault enquiry where at least everyone fesses up to what happened rather than just covers themselves and obfuscates the whole sad scenario.
Let us at least learn all we can from an "accident".
Good judgment comes from experience, experience comes from bad judgment!
