The German health insurance system is hideously complicated, so I don't want to get into too much detail, but what it comes down to is that back in the day, companies were allowed to offer their employees health insurance as a side benefit. By now, those company specific insurers are pretty much open to anybody and compete with regular insurers. So the only real difference now is the name.
